Abstract:

Background: Acute Gouty Arthritis is a metabolic inflammatory arthropathy driven by hyperuricemia and monosodium urate (MSU) crystal deposition, causing severe joint pain, swelling, and redness. Linked to dietary imbalance, obesity, and metabolic disorders, its presentation closely resembles Paithika Vatarakta in Ayurveda, involving Pitta, Vata, and Rakta.

Aim: To correlate the pathophysiology, diagnosis, and management of Acute Gouty Arthritis with Paithika Vatashonita.

Methods: We reviewed classical texts like the Brihat Trayi (e.g., Charaka Samhita Ch. 29) and Laghu Trayi (e.g., Madhava Nidana Ch. 23) alongside modern medical history from Hippocrates to Garrod [16]. Recent clinical trials (2015–2025) on treatments like Kokilaksha Ghanavati and Guduchi Kashaya were analysed for their effects on swelling, pain, and uric acid. We compared modern crystal-induced pathology with the Ayurvedic concept of Avarana, where polluted Rakta blocks Vata to cause joint inflammation.

Results: Modern science identifies MSU crystal deposition and IL-1β-mediated inflammation, whereas Ayurveda describes Paithika Vatashonita through vitiated Rakta and aggravated Vata-Pitta, producing Daha (burning), Shotha (swelling), and Vedana (pain). Concepts like Ama and Vata-Rakta Sammurchana align seamlessly with the modern inflammatory-metabolic framework.

Conclusion: Shared etiological factors, pathogenesis, and clinical features confirm a strong conceptual correlation between Acute Gouty Arthritis and Paithika Vatarakta, offering a robust Ayurvedic framework for understanding and managing acute gout flares.
